Job Summary:

The Pharmacist position within our pharmacy department aims to ensure safe, effective, and cost-efficient drug therapy. Responsibilities include preparing and distributing medications in compliance with federal and state laws, providing patient counseling and education, and advising medical and nursing staff on drug therapy. The role also involves maintaining an appropriate drug inventory and participating in quality improvement initiatives that directly impact patient healthcare. Adherence to confidentiality and professionalism is essential, in line with the Standards for Service Excellence outlined in the CLIENT Human Resources Manual.


Key Essential Functions and Accountabilities:

  • Prescription Review: Evaluate all prescriptions for appropriateness, legality, completeness, contraindications, interactions, allergies, adverse drug reactions, and lab value abnormalities.
  • Clinical Consultation: Provide clinical consultation to prescribers, resolving any concerns prior to dispensing.
  • Documentation: Accurately document all clinical activities and interventions, entering prescriptions into pharmacy software.
  • Order Verification: Timely verify and fill prescription orders while ensuring all record-keeping and documentation standards are met.
  • Patient Counseling: Advise patients on medication use, including drug name, strength, indications, and expected outcomes, ensuring patient understanding.
  • Chart Reviews: Conduct reviews for prescription refills and medication therapy management; recommend actions to providers and request refills as necessary.
  • Drug Information: Provide drug information, therapy consultation, and staff education regarding drug therapy.
  • Team Leadership: Direct pharmacy support personnel and verify the daily activities of pharmacy technicians.
  • Quality Improvement: Participate in clinical and administrative activities, quality control, and quality improvement initiatives.
  • Policy Formulation: Contribute to policy and procedure development affecting departmental operations.
  • Health Promotion: Engage in health promotion and disease prevention activities related to drug use.

Education, Certifications, and Licenses Required:

  • Education: Pharm.D. degree and licensure as a registered pharmacist. Candidates licensed before 2003 with a Bachelor of Pharmacy degree and three years of relevant experience may be considered.
  • Licensure: Unrestricted license as a registered pharmacist in any state.
  • Certifications: Basic Life Support for Healthcare Providers (CPR and AED) certification (can be obtained within one year of hire).
  • Residency: Completion of a nationally recognized pharmacy residency program (ASHP) is preferred.

Knowledge, Skills, and Abilities:

  • Proficiency in reviewing patient medical histories and medication orders, with strong knowledge of pharmaceutical sciences.
  • Understanding of state and federal pharmacy laws.
  • Ability to provide age-appropriate care to patients of all ages.
  • Strong communication skills, both written and verbal, to engage with patients and staff effectively.
  • Proficient in computer applications and pharmacy software.
  • Strong math skills for accurate drug dosage calculations and compounding formulations.
  • Ability to work collaboratively within the healthcare team while exercising sound professional judgment.
  • Capacity to handle interruptions and adapt to changing workloads.
  • Independence in setting priorities and making critical decisions quickly.
