Certification Requirements:

  • License: Active Arizona or Compact license required.
  • Certifications: Must be current and accurate.

Job Details:

  • Start Date: ASAP - 10/7
  • Department: Pharmacy | Pharmacist
  • Emergency Medication Protocol: ACLS preferred; attendance at codes is required.
  • Experience Required:
    • Familiarity with Vancomycin AUC dosing.
    • Proficient in EPIC, Lexicomp, Baxter IQ.
  • Shifts: Day and evening shifts needed; current day availability is essential.
  • Weekend Requirements: Every other weekend or one per month preferred.
  • Holiday Requirements: 2 out of 3 holidays required.
  • Medication Dispensing System Experience:
    • Omnicell
    • Central pharmacy robot
    • Carousel
    • Omnicell XT cabinets
    • Omnicell Controlled Substance cabinet
  • Dress Code: Business casual/white coat or khaki scrubs.
  • Orientation: Includes one NEO, one clinical education/EPIC session, and one day on the floor.
  • Schedule: 7 on/7 off, 10-hour days (Thursday - Wednesday), with evening shifts as needed.

Job Requirements:

  • Experience: Must have 2 years of hospital pharmacy experience in a facility of similar size (250 census).
  • Resume Requirements: Must detail duties, hospital size, unit size, and trauma level (if applicable) for each position held.
  • Employment Gaps: Explanation required upon submission.

Schedule Information:

  • Work Schedule: 7 on/7 off, 10-hour days (Thursday - Wednesday), with evening shifts as needed.
  • On-Site Support: One clinical manager on-site 5 days a week.
  • Weekend Requirements: Every other weekend or one per month preferred.
  • Holiday Requirements: 2 out of 3 holidays required.

Unit Specific Information:

  • Department-Specific Orientation: One NEO, one clinical education/EPIC session, and one day on the floor.

Additional Details:

  • ACLS Certification: Preferred; will attend codes.
  • Preference for: IV pharmacist/compounding pharmacist.
  • EPIC Experience: Required with no inconsistencies between resume and cover letter.
  • EMR: Must be listed on each work assignment.
  • Time Off Requests: Limited to 5 days or less and included within the attached profile/resume.
