About the Role

St. Elizabeth Healthcare is seeking a highly skilled Specialty Pharmacy Clinical Pharmacist to deliver direct patient care through collaborative care agreements. This role involves optimizing specialty medication therapies for diverse disease states, enhancing outcomes, safety, and cost utilization.

Key Responsibilities

Direct Patient Care:

  • Collaborate with practitioners to manage specialty medication therapies for conditions such as:
    • Dermatology
    • Endocrinology
    • Hepatitis C
    • Inflammatory Bowel Disease
    • HIV and HIV PrEP
    • Rheumatology
    • Hyperlipidemia
    • Osteoporosis
    • Pulmonology
    • Neurology

Pharmacy Operations:

  • Review and monitor treatment medications for appropriateness using clinical guidelines.
  • Verify prescription filling and offsite facility orders, ensuring timeliness and accuracy.
  • Counsel patients on medications and recommend over-the-counter products where appropriate.
  • Maintain and manage inventory, adhering to sound accounting principles.

Leadership & Supervision:

  • Supervise and mentor pharmacy interns and technicians.
  • Ensure the completion of tasks and adherence to department values (ICARE).
  • Act as a liaison with internal departments, including Specialty Pharmacy, Hospice, and Meds-to-Beds programs.

Professional Standards & Competency:

  • Stay updated with current pharmacy practices and maintain licensure.
  • Adhere to regulatory guidelines (State Board of Pharmacy, DEA, CMS, FDA, Joint Commission).
  • Complete all required departmental competency assessments and training.

Additional Duties:

  • Provide timely drug information to medical and nursing staff.
  • Support the specialty pharmacy through on-call duties as needed.

Qualifications

Education & Credentials:

  • Graduate from an NABP-accredited college of pharmacy.
  • Current pharmacist license in the state of employment.

Preferred Experience & Skills:

  • Board Certification in Ambulatory Care Pharmacy or completion of a PGY2 residency in Ambulatory Care Pharmacy.
  • Minimum of 3 years of experience in a similar setting.
  • Knowledge of ASHP standards and problem-solving skills.
  • Strong multitasking, communication, and customer service skills.
