Position Overview

The Ambulatory Clinical Pharmacist plays a vital role in providing direct patient care through collaborative care agreements for various disease states, including dermatology, endocrinology, hepatitis C, inflammatory bowel disease, HIV, rheumatology, pulmonology, neurology, and more. This role focuses on optimizing specialty medication therapies to enhance patient outcomes, safety, and cost efficiency.

The Outpatient Pharmacist ensures accurate prescription fulfillment and provides comprehensive counseling on prescription and over-the-counter medications. The position includes responsibilities for inventory management, staff supervision, regulatory compliance, and serving as a liaison between pharmacy and other departments.


Key Responsibilities

  • Deliver exceptional customer service using the AIDET model.
  • Review and monitor patient treatments for appropriateness based on clinical and institutional guidelines.
  • Perform final verification of prescription filling and offsite facility orders for accuracy and timeliness.
  • Provide timely and accurate drug information to patients and healthcare professionals.
  • Supervise and mentor pharmacy interns and technicians, ensuring adherence to ICARE values.
  • Manage inventory and ordering, maintaining sound financial and accounting practices.
  • Act as a liaison to departments such as hospice, social services, specialty pharmacy, and provider offices.
  • Maintain professional competency, licensure, and knowledge of current pharmacy practices.
  • Ensure compliance with regulatory guidelines (State Board of Pharmacy, DEA, CMS, FDA, The Joint Commission).

Required Qualifications

  • Education: Graduate from an NABP-accredited pharmacy college.
  • Licensure: Current U.S. pharmacist license in the state of employment.
  • Specialized Knowledge:
    • Problem-solving and critical thinking skills.
    • Multitasking ability.
    • Excellent customer service and communication skills.
    • Familiarity with ASHP Current Practice Standards.
  • Experience:
    • Previous pharmacy experience.
    • Board Certification in Ambulatory Care Pharmacy, PGY2 pharmacy residency in Ambulatory Care Pharmacy, or three years of relevant experience in a similar setting preferred.

Physical Requirements

  • Able to perform prolonged periods of sitting, standing, and walking.
  • Proficient in communicative, auditory, and visual skills.
  • Ability to lift/push/pull up to 20 lbs.
