Position: Clinical Pharmacist
Location: Lake Health District, Oregon


Position Overview

The Clinical Pharmacist plays a critical role in ensuring safe, effective, and prompt medication therapy for patients in the Lake Health District. This professional collaborates with interdisciplinary teams to optimize medication use systems, deliver individualized pharmaceutical care, and enhance overall patient outcomes.


Key Responsibilities

  • Provide direct patient care and medication therapy management in collaboration with physicians, nurses, and hospital personnel.
  • Address a wide range of disease states, tailoring drug therapies based on age, disease specifics, and patient response.
  • Deliver pharmaceutical care, including patient education and clinical interventions to prevent medication adverse events.
  • Monitor, evaluate, and enhance medication-use processes through quality improvement initiatives.
  • Conduct medication and pharmacy practice education/training for staff.
  • Participate in the hospital’s quality management program, setting and tracking long- and short-term quality improvement goals.
  • Serve as a resource for internal departments and external groups on medication use and pharmacy practices.

Requirements

Education:

  • Graduate of an ACPE-accredited College of Pharmacy.

Licensure/Certification:

  • Eligible for or currently licensed in the state of Oregon.

Experience:

  • Preferred: Clinical pharmacy experience in a hospital setting.

Employment Requirements

  • Submission of application, cover letter, and resume, detailing employment gaps and reasons for separation.
  • U.S. Citizenship or National status.
  • Satisfactory adjudication of background investigation and/or fingerprint check.
  • Completion of a 500-hour probationary period.
  • Pre-employment drug screening and criminal background check.

Veteran’s Preference

  • Submit DD214 Certificate of Release from Active Duty to claim preference.
  • If currently on active duty, provide certification of expected discharge under honorable conditions within 120 days.
  • For 10-point preference, additional proof of entitlement is required.
