Job Summary

We are seeking a highly motivated and detail-oriented Remote Pharmacist to join our team. The ideal candidate will be responsible for providing comprehensive pharmaceutical care, ensuring the safe and effective use of medications, and supporting our patients and healthcare providers through various channels.

Key Responsibilities

  • Medication Review: Conduct thorough reviews of patient medication regimens to ensure safety, efficacy, and adherence.

  • Patient Counseling: Provide expert advice and counseling to patients regarding medication use, potential side effects, and interactions.

  • Drug Information: Serve as a resource for drug information, answering inquiries from patients and healthcare professionals.

  • Medication Therapy Management (MTM): Perform MTM services to optimize therapeutic outcomes for patients.

  • Quality Assurance: Participate in quality assurance activities to ensure compliance with regulatory standards and best practices.

  • Education and Training: Develop and deliver educational materials and training sessions for patients and healthcare providers.

  • Documentation: Maintain accurate and up-to-date patient records and documentation.

  • Collaboration: Work closely with other healthcare professionals to coordinate patient care and ensure optimal outcomes.

Qualifications

  • Education: Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.

  • Licensure: Active pharmacist license in good standing.

  • Experience: Minimum of 2 years of experience in a clinical or community pharmacy setting.

  • Skills: Excellent communication, problem-solving, and organizational skills. Proficiency in using pharmacy management software and telehealth platforms.

  • Attributes: Strong attention to detail, ability to work independently, and a commitment to patient-centered care.

Compensation
