Job Title: Inpatient Clinical Coder IV
Location: Fully Remote
Job Type: Full-time, Permanent
Shift: 8 AM to 5 PM
Salary Range: $30.00/hr. to $45.00/hr.


Job Summary

As an Inpatient Clinical Coder IV, you will be responsible for reviewing clinical documentation and diagnostic results to extract and abstract data, applying the appropriate ICD-9-CM/ICD10-CM/PCS and CPT 4 codes. Your role ensures accurate reimbursement, supports external reporting, and complies with regulatory requirements, including medical necessity, CCI, and NCCI edits. You will code and abstract complex medical records within the Primary Enterprise acute care facilities.

Essential Functions

  • Code Assignment: Review high-complexity medical records to determine and assign the appropriate principal diagnosis, procedure codes, and all relevant secondary diagnoses and procedure codes. Assign Present on Admission, Hospital-Acquired Condition, and Core Measure Indicators for all diagnosis codes.
  • DRG and APC Facilitation: Assign the correct MS-DRG for inpatient records and APC for outpatient records, following UHDDS and facility guidelines.
  • Technical Proficiency: Use the facility encoder as it interfaces with the hospital mainframe and/or EMR, whether on-site or remotely.
  • Charge and E/M Review: Review charges and Evaluation and Management levels.
  • Data Abstraction: Abstract coded data and other pertinent information into the hospital electronic health record, ensuring data accuracy.
  • Quality and Productivity: Meet established quality and productivity standards.
  • Peer Review and Training: Facilitate peer reviews and provide training for Acute Clinical Coders within the department, offering support to management.
  • Regulatory Compliance: Stay updated on coding principles and regulatory guidelines related to inpatient and/or outpatient coding.

Physical Requirements

  • Must be able to concentrate and sit for extended periods while reviewing electronic health records.
  • Ability to meet daily and weekly deadlines in a fast-paced office or home environment.

Education, Experience, and Certifications

  • Education: High school diploma or GED required; Bachelor’s degree preferred.
  • Knowledge: Advanced knowledge in Medical Terminology, Anatomy, Physiology, and Pharmacology required.
  • Experience: Minimum of 4 years coding experience in an acute care setting required.
  • Certifications: Current RHIA, RHIT, CCS, CPC-H, CPC, or CIC certification required, along with a passing score on the Coding test.
