Position Title: Nurse Practitioner - Behavioral Health

Location: Remote

Position Summary: The Nurse Practitioner will join our team to provide behavioral health integration services, oversee care plans, and deliver psychiatric and cognitive assessments. This role involves coordinating care, improving patient outcomes, and supporting caregivers through advanced care planning and clinical support.

Key Responsibilities:

  • Behavioral Health: Conduct initiating visits for members with behavioral or psychiatric conditions requiring Behavioral Health Integration (BHI) services.
  • Care Plan Review: Review care plans developed by clinical staff, focusing on behavioral health counseling and education.
  • Routine Psychiatric Services: Provide psychiatric services, review clinical status, and offer care recommendations.
  • Cognitive Assessments: Perform cognitive assessments for suspected cognitive impairments and ensure comprehensive patient care.
  • Care Coordination: Recommend cost-effective, research-based treatments and interventions, and coordinate care efforts.
  • Clinical Skills: Apply advanced clinical skills in physical assessment and chronic disease management for at-risk adults, and develop individualized care plans.
  • Advanced Care Planning: Engage in discussions about advanced care planning with caregivers and members.
  • Meetings and Committees: Attend meetings, training sessions, and participate in committees as required.
  • Quality Control: Maintain knowledge of clinical procedures and standards, and monitor care quality.
  • Education: Provide education to members, caregivers, and facilities.
  • Plan Updates: Monitor and update care plans to ensure quality care.

Qualifications:

  • Licensure and Certification: Certified and licensed as a Nurse Practitioner in New York with ANCC or equivalent certification in Psychiatric Mental Health, and Primary Care, Family, Adult/Geriatric, or Acute Care.
  • Education: Master’s or Doctoral degree from an accredited institution for nurse practitioners.
  • Experience: At least 1 year of Nurse Practitioner experience with the ability to practice independently; 2+ years working with complex adult populations preferred.
  • Assessment Skills: Experience in performing cognitive assessments and administering tools such as PHQ2s, PHQ9s, and GAD-2 assessments.
  • Medicare Participation: Ability to have services billed on your behalf.
  • Technology: Comfort with technology such as Google Suite, multiple EMRs, and Slack; experience in home care, family medicine, or geriatrics preferred.
  • Bilingual: English and Spanish language skills preferred.
  • Remote Work: Ability to work remotely with a private workspace and computer.
