Schedule:

  • Monday to Friday: 7:00 am - 3:00 pm, 10:30 am - 6:30 pm, 11:30 am - 7:30 pm (rotating shifts)
  • Rotating weekends: Saturday 8:00 am - 4:00 pm, Sunday 8:00 am - 2:00 pm

Responsibilities:

  • Evaluate physician medication orders for appropriateness, including drug selection, dosage, potential interactions, and route of administration, and make recommendations as necessary.
  • Maintain an up-to-date patient/resident database with clinical and laboratory data, as well as concurrent medications, including over-the-counter products.
  • Ensure appropriate laboratory tests are ordered for high-risk conditions and collaborate with healthcare teams to monitor and discuss findings.
  • Document all communications with physicians, nurses, and healthcare staff in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, and disposal of drugs.
  • Inform the Pharmacy Manager of any medication-related errors and assist staff with safe medication practices.
  • Perform other duties as assigned.

Qualifications:

  • Bachelor's degree in Pharmacy; Pharm D preferred
  • Minimum of one year of experience in long-term care
  • Licensure as required by state and regulatory agencies
  • Detail-oriented, goal-driven, team player with strong oral communication skills
