Primary Function/General Purpose of Position

The Clinical Pharmacist dedicates 50-90% of their schedule to clinical programs and initiatives. This role ensures safe, rational, and cost-effective therapy while adhering to Federal, State, and accrediting agency standards.


Essential Job Functions

  • Conduct medication management activities focused on specific clinical areas or patient populations.
  • Participate in clinical programs to enhance patient care, optimize medication use, and support health and disease prevention initiatives.
  • Perform medication reviews and therapy management to achieve defined patient outcomes and ensure safe medication use.
  • Support pharmacy operations, ensuring accurate and timely medication distribution.
  • Promote cost-effective medication utilization and resource management.
  • Collaborate with pharmacy team members, healthcare professionals, and leaders to deliver high-quality pharmaceutical services.
  • Identify, document, and resolve potential or actual adverse medication events; may analyze data trends for prevention.
  • Provide education to patients and associates to improve understanding and compliance with medication therapies.
  • Educate pharmacy staff and serve as a preceptor for students and residents as assigned.

Licensing/Certification

  • Required:

    • Current pharmacist license in the state(s) of practice
    • Valid state-issued ID
  • Preferred:

    • BLS (Basic Life Support)
    • ACLS (Advanced Cardiovascular Life Support)
    • Immunization Certification

Education

  • Required: Graduate of an ACPE-accredited School of Pharmacy (BS or PharmD)
  • Preferred: Advanced pharmacy degree (e.g., PharmD, Masters, Residency)

Work Experience

  • Required:

    • Advanced knowledge for pharmaceutical assessments and patient management plans
  • Preferred:

    • Pharmacy residency or three years of equivalent experience
    • Board certification (e.g., BCPS)

Training

  • Proficient in computer hardware and software applications

Patient Population

Demonstrates the ability to provide age-appropriate care, adhering to policies and procedures for the following groups:

  • Neonates (0-4 weeks)
  • Infants (1-12 months)
  • Pediatrics (1-12 years)
  • Adolescents (13-17 years)
  • Adults (18-64 years)
  • Geriatrics (65+ years)

Working Conditions

  • High-stress and variable workload periods may occur.
  • General office environment with exposure to limited hazardous substances, body fluids, or infectious materials.
  • Possible travel to off-site locations in adverse weather conditions.
  • Prolonged periods of working alone.

Universal precautions and personal protective equipment (PPE) are required for infection control.


Physical Requirements

  • Manual Dexterity: Eye/hand coordination, repetitive arm/hand movements, reaching above shoulder
  • Physical Activities: Prolonged standing, walking, or sitting
  • Hazards: Exposure to chemicals, mechanical parts, dust, and high-pitched noises
