Pharmacist Opportunity with Sutter Health - San Francisco, CA

Position Overview:
Join the Sutter Sync program with Sutter Health, a comprehensive care management program designed to manage patients with chronic conditions such as hypertension or diabetes. Using remote patient monitoring tools and various technologies, you will collaborate with Health Coaches to manage a panel of patients, adjust medications, and provide personalized lifestyle advice. Initially, you’ll be required to work onsite at the Innovation Center in San Francisco for the first few weeks or months, after which there will be an opportunity to work remotely. This role is part of a growing team and will involve shaping workflows and creating onboarding materials for future pharmacist hires before the program's launch in March 2025.

Key Responsibilities:

  • Oversee and deliver pharmaceutical care through patient-centered clinical services, including medication information, education, preparation, and distribution.
  • Collaborate with the healthcare team to adjust patient plans of care.
  • Maintain patient safety and privacy while adhering to local/state/federal regulations.
  • Utilize clinical expertise to solve complex patient care problems and support patient education.

Education and Certifications:

  • Doctorate in Pharmacy from an accredited college of pharmacy.
  • Licensure: Licensed Pharmacist (PHARMD).

Experience:

  • 3 years of recent, relevant experience in pharmacy practice.
