Description

Brief Summary:

Provide accurate, safe, efficient, and cost-effective pharmaceutical care to patients of Samuel Simmonds Memorial Hospital and retail consumers, either directly or through the supervision of support staff.

Essential Job Functions:

  • Serve as a specialist in an assigned area of expertise, engage in consumer education, and provide clinical practice direction to other staff members.
  • Assist in developing, implementing, and evaluating treatment guidelines for cost-effective and appropriate drug therapy.
  • Participate in interdisciplinary groups to enhance patient care.
  • Contribute to Performance Improvement, Drug Utilization Evaluation, Adverse Drug Reaction reporting, and other projects as assigned by the Director of Pharmacy.
  • Coordinate and evaluate staff development opportunities for Pharmacy Technicians, including assessing their clinical skills and providing performance review input.
  • Conduct and participate in clinical research.
  • Provide accurate and efficient pharmaceutical distribution services by interpreting provider drug orders, entering them into the pharmacy system, and dispensing medications in accordance with departmental protocols.
  • Identify, resolve, and prevent potential drug-related issues by evaluating medical data, including patient history and laboratory results, to implement optimal drug therapy.
  • Communicate with physicians, nurses, and other healthcare providers about patient drug therapy.
  • Document and communicate drug information provided to patients and/or their representatives.
  • Monitor patient drug therapy for cost-effectiveness and share cost-saving recommendations with physicians.
  • Maintain and update personal drug knowledge to ensure accurate pharmaceutical care, keeping abreast of general disease states and specialized areas of expertise through publications, presentations, and professional development.
  • Document all medication errors and near misses in the Quatro system.
  • Stay current with state and federal regulations governing pharmacy practice, ensuring compliance in all professional activities.
  • Supervise support personnel in accordance with state laws, ensuring accurate preparation and dispensing of pharmaceuticals, and take responsibility for the pharmaceutical and clinical services provided by supervised staff.

Education & Experience:

  • A Bachelor's degree in Pharmacy is required; an advanced degree (Pharm.D.) is preferred.
  • Completion of a residency and a minimum of two years of post-graduate clinical practice experience or equivalent in retail and inpatient pharmacy is preferred.
  • Advanced Cardiac Life Support certification and active membership in state and national pharmacy organizations are preferred.

Certification/Licensure:

  • Must hold a valid, unrestricted license to practice pharmacy and dispense controlled substances within the United States.
