Pharmacist - Full-Time

Location: Lancaster Family Health Center, Salem, OR
Compensation: $142K - $169K/year (DOE), with higher pay for highly experienced candidates

Overview:

Join the Yakima Valley Farm Workers Clinic as a Pharmacist at the Lancaster Family Health Center in Salem, OR, and be part of a healthcare organization that’s committed to making a difference in the lives of migrant farmworkers and their families. We offer a holistic model of care with a wide range of services across 40 clinics in Washington and Oregon, focusing on the health, well-being, and community of our patients.

Responsibilities:

  • Patient Care: Provide services like drug utilization review, patient counseling, drug regimen review, and medication therapy management.
  • Collaborative Drug Therapy Management: Work with healthcare providers to manage post-diagnostic disease states.
  • Prescribing & Testing: Prescribe medications or devices and order, interpret, and monitor laboratory tests, as per protocols.
  • Pharmacy Oversight: Supervise pharmacy personnel, ensuring they follow their scope of practice and maintain the security of the pharmacy.
  • Compliance & Records: Ensure all prescriptions are dispensed accurately in accordance with state and federal laws, and maintain proper records.
  • Education & Development: Stay updated with pharmacological knowledge by attending educational workshops and participating in professional societies.
  • Clinical Responsibilities: Perform duties in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.

Qualifications:

  • Education: Bachelor’s Degree in Pharmacy required, Pharm D preferred.

  • Experience: Community or ambulatory pharmacy experience preferred. PGY-1 residency = 2 years of work experience, PGY-2 = 3 years.

  • Licensing: Current OR State Pharmacist License required.

  • Skills:

    • In-depth knowledge of community pharmacy practice and drug therapy.
    • Strong attention to detail and customer-oriented communication skills.
    • Ability to prioritize and multitask in a fast-paced environment.
    • Proficiency with computer systems, including Microsoft Office and pharmacy software.
    • Knowledge of State and Federal Pharmacy Laws.
  • Drug Testing: Required for controlled substances before employment, with periodic testing throughout employment.
