Your Role

As a Pharmacist at Salem Health, you will oversee pharmacy technicians, interns, and residents, ensuring accurate preparation and dispensing of medications. You will evaluate patient records, manage medication therapy, and provide expert consultation to healthcare providers and patients. Additionally, you will play a critical role in inventory management, compliance, and educational mentorship.


Core Functions

Patient Care and Medication Management

  • Evaluate electronic medical records for clinical and laboratory data, medical history, and concurrent medications.

  • Review and verify medication orders for appropriateness, potential interactions, dosage, and stability.

  • Participate in unit care rounds and consult with medical teams regarding drug therapy options.

  • Monitor and manage ongoing medication therapy to optimize therapeutic results.

  • Document therapeutic interventions and resolve medication-related issues with prescribers.

Supervision and Oversight

  • Direct and supervise pharmacy technicians, interns, and residents.

  • Verify the accuracy of medication preparation and dispensing activities.

  • Conduct inspections and maintain medication inventories aligned with patient care needs.

Collaboration and Education

  • Serve as a liaison to nursing units or departments to ensure proper medication use.

  • Provide medication information and pharmacokinetic dosing advice to medical personnel.

  • Mentor and support pharmacy trainees and students.


Qualifications

Education and Licensure

  • Doctor of Pharmacy (PharmD) required.

  • Licensed Pharmacist in Oregon required.

  • Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications required.

Preferred Experience

  • Completion of a PGY1 Pharmacy Residency.

Skills and Knowledge

  • Strong clinical knowledge of medication therapy management and pharmacology.

  • Proficient in evaluating medical records and collaborating with interdisciplinary teams.

  • Familiarity with Lean principles for process improvement is a plus.
