Your Role With Us

As an Outpatient Pharmacist at Salem Health, you'll play a vital role in supervising pharmacy technicians, interns, students, and residents while ensuring the safe and effective use of medications for hospitalized or ambulatory patients. You will collaborate with medical providers and healthcare teams to manage medication therapy, provide patient consultation, and oversee the accurate preparation, storage, and dispensation of pharmaceutical products.


Core Responsibilities

Patient Care and Medication Management

  • Evaluate electronic medical records, including patient history, clinical and lab data, and concurrent medications.
  • Participate in Unit Care Rounds and review patient interviews for medication history, allergies, and sensitivities.
  • Verify provider medication orders for accuracy, dosage, stability, and potential interactions, recommending adjustments as necessary.
  • Monitor and manage medication therapy to ensure optimal therapeutic results and cost efficiency.
  • Communicate with prescribing physicians to resolve issues and document interventions.

Medication Preparation and Supervision

  • Order and adjust dosages, order lab tests, and administer medications under protocol per established policies.
  • Oversee and verify the preparation and dispensing of medications by pharmacy personnel.
  • Ensure proper delivery and administration of medications, maintaining high standards of accuracy.

Collaboration and Education

  • Serve as a liaison to nursing units or departments using or stocking medications, establishing inventory and par levels as needed.
  • Supervise Certified Pharmacy Technicians, Pharmacy Interns, Students, and Residents.
  • Provide education and consultation to medical personnel on medication information and drug therapy options.

Qualifications

Required

  • Doctor of Pharmacy degree.
  • Licensed Pharmacist in Oregon.
  • Basic Life Support (BLS) certification.
  • Advanced Cardiovascular Life Support (ACLS) certification.

Preferred

  • PGY1 Pharmacy Residency experience.
