Compensation: $64.78 to $97.18 per hour (rate dependent on qualifications, professional experience, and/or work location)


About the Role

PeaceHealth seeks a Pharmacist for a Per Diem/Relief Day Position to provide pharmaceutical care services, including medication review, monitoring, and counseling. The pharmacist will support clinical care through effective collaboration, ensuring safe and effective medication use.


Key Responsibilities

Medication Management

  • Review medication orders for accuracy and appropriateness, including contraindications, allergies, dose, route, frequency, duplicate therapies, and formulary compliance.
  • Interpret, clarify, and rectify physician medication and IV orders and perform medication reconciliation processes.
  • Dispense outpatient prescriptions, ensuring compliance with all legal and safety standards.

Patient-Centered Care

  • Perform patient counseling on medication use and administration.
  • Prescribe drug therapy under collaborative agreements and approved protocols.
  • Monitor patients on targeted drugs and manage pharmacokinetic dosing protocols.

Clinical Excellence

  • Evaluate drug therapy appropriateness and provide recommendations to clinicians.
  • Detect, monitor, and report adverse drug events and medication errors (EIR).
  • Develop and audit drug protocols, clinical pathways, and drug usage evaluations.

Supervision & Training

  • Supervise and train pharmacy technicians, interns, and externs, potentially serving as a preceptor.

Qualifications

Education

  • Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD) from an accredited program.

Licensing

  • Licensed or eligible for licensure as a Registered Pharmacist in Oregon.

Experience

  • Demonstrated basic clinical pharmacy skills.
  • Proven customer service capabilities and ability to work independently.

Skills

  • Strong communication (verbal and written) and organizational skills.
  • Basic proficiency in computer applications, including MS Office.
  • Analytical and mathematical aptitude.

Additional Requirements

  • Basic Life Support (BLS) certification required within six months of hire.
  • COVID-19 vaccination or appropriate exemption as per PeaceHealth policies.
