Position Summary:
The primary functions of this role include assessing, planning, delivering, coordinating, teaching, and providing patient disease management care/treatment in areas such as anticoagulation, hepatitis C, lipid management, smoking cessation, and vaccinations. The position may also assume charge responsibility on occasion and will serve as a preceptor for new hires and students, acting as a resource for other staff.

Requirements:

Minimum Qualifications:

  • Education:
    Graduate of an ACPE-accredited School of Pharmacy. Bachelor of Science in Pharmacy or Doctor of Pharmacy required.

  • Certifications/Licenses:

    • Licensed Registered Pharmacist (RPh) in the State of New Mexico.
    • Licensed Pharmacist Clinician (PhC) by the New Mexico Board of Pharmacy.
    • Registered as a Pharmacist Clinician with the Board of Medicine to exercise prescriptive authority according to current guidelines or protocols defined by the Pharmacist Prescriptive Authority Act, Section 61-11B-1 through 61-11B-3 NMSA and 16.19.4.17 NMAC-RN.
    • Certification in Basic Life Support for Healthcare Providers (BLS).
    • Advanced training/certification in anticoagulation and/or disease management.
  • Skills:

    • Ability to gather and analyze physiological, psycho-social, and socio-economic data.
    • Accurate evaluation of patient conditions in person or via phone.
    • Recognition of changes in patient status and ordering appropriate diagnostic tests and therapies.
    • Effective education, communication, and listening skills with patients, families, physicians, and other staff.
    • Fluency in English required; Spanish fluency preferred.
    • Strong verbal, written, and interpersonal communication skills.
    • Ability to work collaboratively with others and manage clinical pharmacy tasks under stress.
    • Skills in physical assessment, planning, delivery, and evaluation of patient care.
  • Experience:
    Direct patient care experience as a Pharmacist Clinician with prescriptive authority. Current experience in disease management.

Nature of Supervision:
Responsible to the Anticoagulation Management Services Coordinator and Pharmacy Director.

Environment:
May be exposed to physical injury, toxic substances, infectious diseases, and bloodborne pathogens. Requires knowledge of safety standards to limit exposure to infectious diseases.

Physical Requirements:
Ability to bend, stoop, lift up to 20 lbs., reach above head level and below knee level, and move around the clinic.
