Hourly Compensation: $64.78 – $97.18 (based on qualifications, professional experience, and work location)

Position Overview:

PeaceHealth is looking for a Per Diem/Relief Pharmacist to join our team in Florence, Oregon. In this role, you will be responsible for reviewing and interpreting medication orders, evaluating drug therapies, and making recommendations to ensure optimal patient care. You will also be involved in prescribing medications, dispensing prescriptions, and performing medication counseling for patients. Additionally, you will supervise pharmacy technicians and interns, and contribute to the development of drug protocols and clinical pathways.

Key Responsibilities:

  • Medication Review: Ensure appropriateness and accuracy of medication orders, including contraindications, allergies, doses, routes, frequencies, and formulary status.
  • Prescription Clarification: Interpret and rectify physician medication and IV orders, and ensure proper medication reconciliation.
  • Pharmacokinetic Monitoring: Monitor patients on specified target drugs and implement pharmacokinetic dosing protocols.
  • Clinical Recommendations: Evaluate drug therapies for appropriateness and make recommendations to clinicians.
  • Prescribing: Prescribe drug therapy per collaborative agreements and protocols.
  • Medication Dispensing: Dispense medications and provide counseling to patients on proper usage.
  • Adverse Drug Event Monitoring: Detect, monitor, and report adverse drug events and medication errors.
  • Supervision & Training: Oversee pharmacy technicians, interns, and externs to ensure accurate order entry and preparation.

What You Bring:

  • Education: Graduate of an accredited School of Pharmacy with a BS or PharmD degree.
  • Experience: Basic pharmacy clinical skills and demonstrated customer service abilities.
  • Licensing: Ability to obtain a Registered Pharmacist license to practice in Oregon.
  • Skills: Strong planning, organization, and communication skills (both verbal and written).
  • Technical Skills: Proficiency with MS Office applications and basic computer skills.
  • Certifications: BLS certification required within 6 months of hire.
