Hourly Compensation: $64.78 – $97.18 (based on qualifications, professional experience, and work location)

Position Overview:

PeaceHealth is seeking a Per Diem/Relief Pharmacist to join our team in Florence, Oregon. In this role, you will be responsible for reviewing medication orders, evaluating drug therapy for appropriateness, making recommendations to clinicians, and managing the medication reconciliation process. You will work with the pharmacy team to ensure safe and effective medication use, including dispensing medications and performing patient counseling. Additionally, you will supervise and train pharmacy technicians and interns while participating in the development of drug protocols and clinical pathways.

Key Responsibilities:

  • Medication Review: Ensure appropriateness and accuracy of medication orders, including contraindications, allergies, dose, and formulary status.
  • Medication Interpretation: Clarify and rectify physician medication and IV orders and prescriptions.
  • Pharmacokinetic Dosing: Monitor patients on specified target drugs and initiate dosing protocols.
  • Clinical Recommendations: Evaluate drug therapy and provide recommendations to clinicians.
  • Prescribing: Prescribe drug therapy according to collaborative management agreements and protocols.
  • Dispensing: Dispense medications and counsel patients on proper usage.
  • Adverse Drug Event Monitoring: Detect, monitor, and report adverse drug events and medication errors.
  • Supervision & Training: Oversee pharmacy technicians, interns, and externs to ensure accurate order entry and preparation.

What You Bring:

  • Education: Graduate of an accredited School of Pharmacy with a BS or PharmD degree.
  • Experience: Basic clinical pharmacy skills and demonstrated customer service abilities.
  • Licensing: Current or eligibility to obtain Registered Pharmacist license in Oregon.
  • Skills: Strong planning, organization, communication (verbal and written), and mathematical/analytical skills.
  • Computer Skills: Proficiency with MS Office applications.
  • Certifications: BLS certification within 6 months of hire.
