Position Overview:
The Patient Care Coordinator is a full-time remote position responsible for supporting clinical operations through effective communication with members and providers. The role involves administrative tasks, care coordination, and ensuring seamless service delivery in collaboration with hospitals, clinics, and internal teams.

Schedule:

  • Full-time: 40 hours/week

  • Shift Timing: Monday to Friday, 7:30 AM – 5:00 PM PST (8-hour shifts)

  • Occasional overtime based on business needs

  • Paid training aligned with your schedule

Primary Responsibilities:

  • Coordinate care needs with hospitals, clinics, facilities, and the clinical team

  • Manage referrals, prior authorizations, and intake requests

  • Provide administrative support to ACM nursing staff

  • Assist in discharge planning under ACM nurse direction

  • Generate and maintain system reports as needed

  • Handle inquiries from members and providers

  • Maintain confidentiality of all sensitive information

  • Support referrals to appropriate health programs

  • Fulfill additional duties as assigned by ACM Nurses, Supervisor, or Manager

Required Qualifications:

  • High School Diploma/GED or equivalent experience

  • Must be 18 years of age or older

  • 2+ years of experience in a medical office or related field

  • Proficient with computers and Windows-based applications

  • Understanding of medical terminology

  • Availability for full-time work within the required schedule

Preferred Qualifications:

  • Proficiency with MS Word, Excel, Outlook, and Adobe

  • Experience in the healthcare industry or insurance field

  • Background in clerical or administrative support

  • Prior experience in a call center environment

  • Familiarity with managed care, regulatory guidelines, and government programs

Telecommuting Requirements:

  • Secure and private home workspace

  • Must maintain confidentiality and protect sensitive documents

  • High-speed internet connection approved by UnitedHealth Group

  • Excellent verbal and written communication skills

Compensation & Benefits:

  • Hourly Pay Range: $16.88 to $33.22 (based on experience and location)
