Key Responsibilities:

  1. File Management:

    • Prepare and organize files for patient discharges.
    • Collect discharge files from wards and arrange them systematically in the Medical Record Room.
  2. Report Issuance:

    • Handle the issuance of various reports, including:
      • Pathology reports
      • Radiology reports
      • Cath lab reports
      • Cardiology reports
      • X-ray reports
  3. Certificate Management:

    • Issue certificates such as:
      • Discharge certificates
      • Fitness certificates
      • Expenditure certificates
      • Railway reservation concession certificates
    • Manage the issuance of Mediclaim forms.
  4. Special File Maintenance:

    • Maintain records for:
      • In-Patient (IP) expired files
      • IP Medico-Legal Case (MLC) files
  5. Patient Interaction and Queries:

    • Address queries related to medical records.
    • Complete necessary formalities for handing over case papers to patients.
  6. Library and Statistics:

    • Maintain and issue books from the medical library.
    • Track and report statistics (e.g., admissions, discharges, deaths, occupancy rates).
  7. Government Coordination:

    • Liaise with government agencies for issuing Birth and Death Certificates.
  8. Audit and Training:

    • Conduct monthly random audits to check the completeness of medical records.
    • Provide on-the-job training to staff.
    • Develop and maintain Standard Operating Procedures (SOPs) for the department.
