Position Summary:

The Medical Records Coder II (Inpatient) is a certified coding professional responsible for coding medical records using ICD-10-CM, ICD-10-PCS, and CPT-4 conventions. This role includes coordinating and reviewing the work of junior staff and assisting with training and continuing education programs. The coder will ensure accuracy in diagnosing and procedure coding for optimal reimbursement.


Key Responsibilities:

  • Review complex medical records requiring research and reference checks to accurately code primary and secondary diagnoses and procedures.
  • Oversee the work of designated employees, conducting regular audits to ensure quality and productivity.
  • Assist in developing and presenting continuing education programs in coding and medical record practices.
  • Ensure compliance with coding guidelines, accurately sequence diagnoses and procedures, and verify DRG/APC assignments.
  • Educate physicians on coding practices and work collaboratively with nursing and ancillary staff for accurate documentation.
  • Maintain current knowledge of anatomy, physiology, medical terminology, and coding guidelines through ongoing education.
  • Stay informed about medical record practices, standards, and regulations, including JCAHO and HCFA guidelines.
  • Support special projects and perform other related duties as required.

Required Qualifications:

Education:

  • High school diploma required.

Experience:

  • Certifications:
    • RHIA or RHIT certification: No prior experience required.
    • CCS, CPC, or HCS-D certification: Minimum of two years of relevant coding experience is required.

Certifications:

  • Must hold one of the following active certifications:
    • Registered Health Information Administrator (RHIA)
    • Registered Health Information Technician (RHIT)
    • Certified Coding Specialist (CCS)
    • Certified Professional Coder (CPC)
    • Homecare Coding Specialist-Diagnosis (HCS-D)

Knowledge, Skills, and Abilities:

  • Proficient in ICD-10-CM, ICD-10-PCS, and CPT-4 coding conventions.
  • Strong understanding of anatomy and physiology, medical terminology, and DRG/APC reimbursement systems.
  • Familiarity with coding software and data entry processes.
  • Excellent written and verbal communication skills.
