Job Title: Medical Scribe (Hybrid/Remote)

Location: Arizona (Hybrid Model: 2 days remote, 3 days onsite; option for full remote after 90 days with Management approval)

Role Overview:

At Oak Street Health, our Medical Scribes play a crucial role in supporting primary care providers by handling clinical documentation, allowing providers to focus on delivering exceptional patient care. Our Scribes, also known as Clinic Informatics Specialists (CISs), are integral to ensuring accurate and timely clinical documentation. This role involves observing and documenting patient interactions, assisting with next steps post-visit, and contributing to the overall care process.

Responsibilities:

  • Documenting Patient Encounters (80%)

    • Join providers in the exam room to observe and document patient visits.
    • Accurately document patient encounters, including history, assessment, plan, and physical exam.
    • Assign appropriate CPT and ICD-10 codes to each encounter.
    • Prepare After Visit Summaries.
    • Consult with providers to ensure accurate and specific documentation.
  • Clinical Documentation Improvement (10%)

    • Request and review medical records.
    • Utilize Oak Street's population health tools to enhance clinical documentation.
    • Support Daily Huddles and Clinical Documentation Reviews.
    • Collaborate with providers to identify and improve documentation opportunities.
  • Administrative Support (10%)

    • Place orders and referrals.
    • Address various tasks and support the care team with additional responsibilities related to documentation.
    • Perform other duties as assigned.

Qualifications:

  • Knowledge

    • Familiarity with medical terminology and common medications, either through a pre-medical degree or prior clinical experience [required].
    • Prior clinical experience, including shadowing or volunteering [strongly preferred].
    • Previous scribe or transcription experience [preferred but not required].
  • Skills

    • Advanced listening and communication skills [required].
    • Proficiency with computers and ability to learn new technical workflows [required].
    • Fluency in Spanish, Polish, Russian, or other relevant languages [required where applicable].
  • Abilities

    • Adaptability to new workflows and quick learning of new concepts and skills [required].
    • Typing speed of 70+ words per minute [strongly preferred].
    • Ability to follow direction and work effectively as part of a team, including adapting to the provider's working style [required].
    • Self-starter with excellent job attendance and commitment to working onsite [required].
    • Willingness to commit to at least 1 year in the role (2+ years ideal) [required].
    • Ability to work 40-45 hours per week during clinic hours, with predictable hours and breaks [required].
    • Compliance with HIPAA and other Oak Street Health policies [required].
    • US work authorization [required].
