Position Overview:
UnitedHealth Group’s Optum Home & Community Care division is seeking a dedicated Field Based Patient Care Coordinator to support our Senior Community Care (SCC) program. You will be instrumental in facilitating care across nursing homes, assisted living facilities, and senior housing by providing essential coordination support to nurse practitioners (NPs) and physician assistants (PAs). This role contributes to delivering holistic healthcare by addressing physical, mental, and social needs.

Schedule:

  • Full-time, Monday – Friday

  • Business hours: 8:00 AM – 5:00 PM

  • No weekends, nights, or holidays

  • Field-based role covering Pierce and King County, WA and surrounding areas

Training:

  • 2 months of paid training

  • Schedule to be discussed on the first day of employment


Primary Responsibilities:

  • Coordinate member care under the direction of an RN or NP/PA

  • Manage medical chart documentation for new members, post-hospital visits, labs, medications, and quality metrics

  • Retrieve and manage patient medical records post-hospitalizations or medical visits

  • Schedule appointments and arrange member transportation as needed

  • Serve as a liaison to ensure seamless coordination between care providers and patients

Required Qualifications:

  • High School Diploma / GED OR equivalent work experience

  • 3+ years of experience in a clinical or medical records setting

  • Familiarity with medical terminology and EHR platforms

  • Access to reliable transportation and ability to travel up to 100%

  • Must reside within Pierce or King County, WA

  • Willingness to work standard business hours, Monday – Friday


Preferred Qualifications:

  • CNA or MA certification

  • Ability to maintain confidentiality and information security

  • Home work environment capable of ensuring privacy and high-speed internet


Physical Requirements:

  • Ability to travel extensively within designated regions

  • Ability to manage secure documentation and medical records


Compensation:

  • Hourly Range: $19.86 – $38.85

  • Actual compensation depends on local labor market, experience, education, and certification

  • Full benefits eligibility based on employment status
