Position Overview:

As a Coordinator for the Senior Community Care (SCC) team, you will support nurse practitioners and physician assistants in coordinating patient care across various settings, including homes, nursing homes, and assisted living facilities. This is a full-time, field-based position requiring travel within Allen County, IN, Monday through Friday (8:00 AM – 5:00 PM).


Key Responsibilities:

  • Support interdisciplinary care team in patient coordination under RN or NP/PA guidance

  • Manage data entry and member chart updates (e.g., hospital visits, lab results, medications)

  • Obtain medical records from hospitals or physician offices

  • Schedule patient transportation and medical appointments

Compensation & Benefits:

Required Qualifications:

  • High School Diploma / GED or equivalent work experience

  • Minimum 3 years of clinical or medical records experience

  • Proficient with medical terminology and EHR platforms

  • Must be 18+ years old and reside in or near Allen County, IN

  • Reliable transportation and willingness to travel 100% within the field

  • Full-time availability, Monday – Friday, 8:00 AM – 5:00 PM


Preferred Qualifications:

  • CNA or MA certification

  • Ability to maintain document confidentiality

  • Dedicated and private home workspace

  • Access to high-speed internet (approved by UnitedHealth Group or existing setup)
