Job Title: Clinical Pharmacist

Position Overview:
The Clinical Engagement Services Center team provides a range of clinical services, including Adherence, Diabetes management, Fraud Waste & Abuse, Medication Therapy Management, Polypharmacy, Retrospective Drug Utilization, and STARS adherence. The team consists of clinical pharmacists, nurses, certified pharmacy technicians, and customer service representatives, all working collaboratively to meet the operational needs of each clinical service.

Work Hours:
Flexibility is required for rotating shifts between 10:00 AM and 8:00 PM CST, Monday through Friday. Extended hours and occasional weekend shifts may be necessary.

Reporting Structure:
The Clinical Pharmacist will report to the Operations Team Manager and will primarily conduct telephonic consultations with members to perform comprehensive medication reviews, adherence consultations, and improve disease state management. The pharmacist will document care delivered and communicate important information to the members’ primary care providers.

Key Responsibilities:

  • Engage with members via phone to schedule consultations for various clinical programs.
  • Ensure the privacy and confidentiality of patient information in compliance with HIPAA and organizational policies.
  • Utilize prescription claims data to generate clinical recommendations in line with Medication Therapy Management (MTM) and Retrospective Drug Utilization Review protocols, addressing gaps in care, high-risk medications, compliance, drug interactions, and therapeutic substitutions.
  • Assess medication regimens for unnecessary or ineffective medications, inappropriate doses, and adverse drug reactions.
  • Provide patient education on medication regimens and disease states.
  • Document patient care notes and recommendations clearly and efficiently.
  • Stay current with emerging clinical trends.
  • Actively participate in departmental meetings to enhance clinical programs and processes.
  • Adhere to all policies and procedures related to the role.
  • Meet productivity and performance expectations set by management.
  • Support the Clinical Service Engagement team with special projects as needed.
  • Perform other duties as assigned by the Operations Manager.

Required Qualifications:

  • Bachelor’s degree in Pharmacy or PharmD.
  • Current and unrestricted pharmacist license in the state of residence.
  • Strong knowledge of disease state management guidelines and the five core elements of MTM Services:
    1. Medication Therapy Review (Comprehensive and Targeted)
    2. Personal Medication List (PML)
    3. Medication-Related Action Plan (MAP)
    4. Interventions and Referrals
    5. Documentation of Services and Follow-up
  • Experience working independently within a cross-functional team environment with strong organizational skills.
  • Proficiency in navigating approved MTM platforms and managing multiple databases and applications.
  • Flexibility to work rotating shifts between 10:00 AM and 8:00 PM CST, with potential for extended hours and weekend work.

Preferred Qualifications:

  • PharmD.
  • Certification as a Diabetes Care and Education Specialist, Board Certified-Advanced Diabetes Management, ASHP Diabetes Management Certificate, or APhA Patient-Centered Diabetes Care Certificate.
  • Bilingual proficiency (medical terminology) in Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese.
  • Additional pharmacist licenses in good standing in Alabama, Idaho, Kentucky, Louisiana, Maryland, Minnesota, Tennessee, Oregon, or Vermont.
  • 2+ years of experience in pharmacy (retail/community or PBM), though new graduates will be considered.
  • Prior call center experience.
  • Intermediate skills in Microsoft Word, Outlook, and Excel.
  • ACPE MTM Specialist and Certified Geriatric Specialist credentials are a plus.

Work Schedule:
The standard hours for this position will be 10:00 AM to 6:30 PM CST, with a closing shift (11:30 AM to 8:00 PM CST) every other week from Monday to Thursday and every eighth Friday. The office will be closed on UHG holidays (Thanksgiving, the day after Thanksgiving, and Christmas Day).

Candidates must hold a pharmacist license in their state of residence.
