Job Description
Position: Clinical Pharmacist (Exempt Position)

The Clinical Pharmacist enhances the quality of patient care by ensuring safe, appropriate, and cost-effective medication therapies. This involves verifying medication orders, consulting with doctors and nurses on dosing and therapeutic alternatives, and monitoring the metabolic and nutritional needs of patients. The Clinical Pharmacist also assists in developing clinical pathways that include monitoring protocols and compliance for medication orders, participating in quality improvement initiatives, and documenting services. Additionally, the role involves performing chart reviews for the Skilled Nursing Unit to meet State Department of Health and Hospitals requirements. This position relies on experience and judgment to plan and accomplish goals.

Responsibilities

Record Management:

  • Conduct skilled nursing chart reviews as required by the State Department of Health and Hospitals, ensuring proper documentation.
  • Actively participate in Skilled Nursing Multidisciplinary weekly meetings to promote optimal patient outcomes, documented by pharmacist signatures on assessment sheets.
  • Accurately document clinical activities in Clinical Measures.

Consultation:

  • Communicate information on pharmaceutical supplies, drug usage, and compatibility to nursing and medical staff within specified time frames.
  • Advise physicians on drug therapy issues, toxicity, side effects, and appropriate dosing, with 100% documentation accuracy in clinical measures.
  • Participate in developing and delivering education programs for Pharmacy and Patient Care Services, as evidenced by documented attendance.
  • Maintain liaison relationships with medical and nursing staff, evidenced by a lack of complaints.

Compounding & Dispensing Medication:

  • Review charts to ensure medication translation and administration align with physician orders.
  • Maintain compliance with federal and state laws regarding drug dispensing, ensuring 100% adherence.
  • Ensure proper preparation, labeling, and delivery of medications according to physician orders, with no errors recorded.

Ensuring Quality and Cost Effectiveness:

  • Evaluate medication regimens for appropriateness, suggesting modifications to achieve desired patient outcomes.
  • Assess and adjust antibiotic dosing based on renal function and diagnosis for optimal patient results.
  • Perform necessary actions to ensure patient safety and compliance with Joint Commission Standards for anticoagulation therapy, documented accurately in Clinical Measures.

Other Duties as Assigned:

  • Mentor pharmacists in training through practical and didactic experiences, demonstrating the ability to complete assigned tasks.
  • Complete other assigned projects as required.

Qualifications

  • Experience: Completion of a residency program or 3 years of clinical experience.
  • Education: As per licensure requirements.
  • Training: As per licensure requirements.
  • Special Skills: Excellent verbal and written communication skills; requires creative analytical thinking and personal judgment to achieve goals.
  • Licensure: Current license with the Louisiana Board of Pharmacy (RPH).
