Job Description: Clinical Pharmacist
Location: McCullough Hyde Memorial Hospital, Oxford, OH, United States

Job Summary:
Become an essential part of patient care as a Clinical Pharmacist! Join our dynamic multidisciplinary team to provide critical drug therapy management, medication reconciliation, and patient education. As a Clinical Pharmacist, you will collaborate with physicians, nurses, and other healthcare professionals to ensure the safe and effective use of medications, improving patient outcomes.

Key Responsibilities:

  • Patient Care: Review patient information, assess drug therapies, and provide pharmaceutical recommendations to maximize clinical outcomes and minimize risks and costs.
  • Collaborative Communication: Work closely with healthcare professionals to ensure proper medication use and resolve any issues before dispensing.
  • Medication Safety: Promote and ensure the safe use of medications by respecting the seven "rights" of medication administration.
  • Regulatory Compliance: Ensure adherence to all regulatory requirements and organizational policies.
  • Clinical Knowledge and Leadership: Take a leadership role in drug distribution processes, optimize protocols, and contribute to pharmacy-related projects.
  • Team Development: Assist in training, precepting, and supporting new hires, pharmacy students, and residents.

Minimum Job Requirements:

  • Bachelor's Degree in Pharmacy
  • Active pharmacist license in Ohio
  • 1-2 years of pharmacy experience (inpatient or infusion preferred)
  • Working knowledge of Ohio Board of Pharmacy regulations
  • Availability to work Tuesday and Wednesday (7:30 AM - 6:00 PM), with every third weekend and holiday required

  • Ability to work collaboratively in multidisciplinary teams
