Clinical Pharmacist - McCullough Hyde Memorial Hospital

Scheduled Hours:

Every Tuesday and Wednesday from 7:30 AM - 6:00 PM. Required to work every 3rd weekend and holiday.


Job Summary:

As a Clinical Pharmacist at McCullough Hyde Memorial Hospital, you will play a vital role in ensuring safe, effective, and personalized medication therapy for patients. You will collaborate closely with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. Your expertise will guide drug therapy management, medication reconciliation, patient education, and more, while also contributing to department projects and initiatives. This role requires a high level of collaboration, professionalism, and dedication to quality care.


Key Responsibilities:

  • Collaborative Care:

    • Work with multidisciplinary teams, including pharmacy leadership, physicians, nurses, and technicians, to provide effective patient care.
    • Review patient information and provide pharmaceutical recommendations to optimize treatment while minimizing risks.
    • Ensure adherence to the "7 Rights" of patient care and provide education on drug therapies.
  • Pharmaceutical Services:

    • Verify and enter medication orders, supervise technician work, and manage drug distribution with accuracy and timeliness.
    • Maintain and optimize drug inventory, ensure accurate stock levels, and prevent waste.
    • Lead efforts in pharmacy process improvements and participate in protocol development to enhance patient care and cost-efficiency.
  • Patient Safety and Compliance:

    • Monitor for medication errors and risks, resolve issues before dispensing, and ensure compliance with all regulations and organizational policies.
    • Contribute to quality improvement initiatives related to medication use and patient care.
  • Professional Development:

    • Pursue continuous learning in clinical pharmacy practices.
    • Mentor and precept pharmacy students and residents, and assist in orientation and training of new hires.
    • Participate in committees and contribute to the development of clinical pharmacy programs.

Minimum Job Requirements:

  • Education:

    • Bachelor’s Degree in Pharmacy.
  • Licensure:

    • Active pharmacist license in the state of Ohio.
  • Experience:

    • 1-2 years of experience as a pharmacist, with inpatient or infusion pharmacy experience preferred.
  • Skills:

    • Strong interpersonal, analytical, organizational, and problem-solving skills.
    • Ability to collaborate effectively with diverse multidisciplinary teams.

    • Occasionally: 11-50 lbs.
    • Consistently: <10 lbs.
  • Mobility & Activity:

    • Frequent: Reaching, walking, standing, and use of hands.
